However I think the significance of the anticholinergics in "death rattle" study was overplayed. My major concern was lack of a placebo arm. While the published RCT confirmed lack of significant difference between the drugs tested it did not answer whether any of these agents are effective. The Cochrane review last year identified lack of conclusive evidence of effectiveness. While this was a well-done study, an important opportunity was missed to definitively answer this question.
