Samuels Foundation Fellowship Grant Program

Grant Program

The American Academy of Hospice and Palliative Medicine (AAHPM) was awarded a grant by the Fan Fox & Leslie R. Samuels Foundation (Samuels Foundation). The funding was used to partially support accredited fellowship training programs in hospice and palliative medicine focused on training physicians interested in caring for the aging in Greater New York City region. Funding was provided to AAHPM for the 2013 academic year.

Hospice and palliative medicine fellowship training programs accredited by either the Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA) were eligible to apply for partial funding support of individual fellowships in hospice & palliative medicine for the 2013 academic year. While the funding is not open to individual applicants, it goes to pay the salary and benefits of individual fellows.

The goal set forth by the Samuels Foundation Grant Program was to support up to eight fellows committed to the field of hospice and palliative medicine and interested in caring for an expanding population of older adults in the Greater New York City region beyond their training year. By leveraging funds to support a centralized source of fellowship funding, AAHPM can foster the growth of the field of hospice and palliative medicine, thereby addressing one of the most critical components of its workforce strategy.

Applications

The application deadline has now passed.

New York City fellowship programs accredited by either the ACGME or AOA were eligible to apply. After reading the detailed Application Instructions programs were asked to submit the following:

1. Application form
2. Biosketch, not to exceed 4 pages each, for your fellowship program director and 3-5 primary teaching team members of any discipline
3. A letter of support from the Sponsoring Department Chair or institutional equivalent including the institutions commitment of matching grant funds for the remainder of salary and benefits and plans for program sustainability for the future
4. A concise (3 pages, maximum) description of the fellowship curriculum including required rotations, schedule, and didactics
5. Institutional PGY salary and benefits schedule
6. Up to three (3) sample evaluation tools for fellows
