Manager, Palliative Care Program

Job Description:  Oversees the quality, service and cost-effectiveness of care delivered in in-patient, home-based and out-patient palliative care programs in collaboration and partnership with the MD chief and physicians.  Ensures integration of palliative care services across the continuum of care for KP patients. Supervises and coordinates the activities of personnel engaged in providing palliative care and related medical services.  Accountable for creating a culture of compliance, ethics and integrity.  Maintains knowledge of and assures departmental compliance with Kaiser Permanente’s Principles of Responsibility and policies and procedures, and applicable regulatory requirements and accreditation standards.  Responds appropriately to observed fraud or abuse.

  • Excellent interpersonal and written and verbal communication skills.
  • Demonstrated analytical, critical thinking and problem solving skills.
  • Ability to build effective working relationships with physicians and their staff, hospitals/SNF units, vendors, and other departments.
  • Excellent planning and time management skills including the ability to handle multiple tasks at any given time.
  • Ability to make quick decisions based on available information, take appropriate action and stick to decisions.

Required Education, Experience and Training:  3-5 years experience with chronically ill adults in an area such as medical, geriatric, home health  or hospice care

  •  
    BSN and/or Nurse Practitioner in adult medicine, geriatrics, or family medicine (prefer Masters in Nursing and/or NP with palliative care certification)
  • Masters level education in psychology, counseling or social work (includes clinical psychologist, LCSW)
  • Chaplain (minimum bachelors degree, prefer masters)
  • Masters in Public Health or Healthcare Administration
  • Three (3) – five (5) years experience with chronically ill adults in an area such as medical, geriatric, home health  or hospice care  
  • Two (2) - three (3) years experience in program development and policy and procedure development
  • Well organized, detail oriented
  • Strong written and verbal communication skills, including presentation skills
  • Effective interpersonal skills to work with patients, families, community personnel, providers and other medical office and hospital staff
  • Facilitation, conflict resolution and negotiating skills
  • Competency using word processing, spreadsheet, scheduling and presentation software
  • Valid Georgia driver’s license.
  • Complete a Customer Service Assessment

Preferred Education, Experience and Training:  Masters in Nursing or health care related field strongly preferred and or APRN in adult, family or geriatric nursing 

  • One year of experience supervising an interdisciplinary team 
  • Experience with quality improvement methodology
